The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

Insurance Options 

Estimated co-payment at visit

For your convenience, we do direct bill. We just ask you to pay the copayment at your appointment.

All providers and plans are different, and we’ll work our hardest to make sure your claim is as painless as our procedures, but insurance coverage is never guaranteed.

To avoid surprises, consult your policy document or call your insurance provider prior to your appointment.

Whenever possible, we will help you figure out your coverage, but some insurance companies don’t disclose these details with the dental office.

In any case, it’s best to be as informed as possible. Here are a few things to look for:

  • percent coverage
  • your deductible
  • annual maximum
  • procedure and frequency limitations
  • exclusions

Payment in full at visit

Some insurance companies are non-assignable, which means they will not send payment to the dentist. In this case, we ask for full payment at the time of your appointment. We will, however, gladly submit the claim on your behalf and your insurance company will reimburse you directly.

If you do not have insurance, it’s no problem. We will always let you know what your treatment will cost before we start and can help you out with a payment plan.

No or low-interest payment plan

If you do find you have a balance remaining after insurance, or are facing treatment without insurance, we can sit down with you and find a time frame and method of payment that leaves everyone smiling.
